Job Description
Juniper Aged Care seeks an experienced Occupational Therapist to join our Home and Community Care team in Western Australia.
You will primarily manage a community caseload with prospects to participate in the Transition Care Program (TCP) to support older adults after hospital stays.
This part‐time role offers 3 days per week, working north of the river, with competitive hourly pay and generous perks.
You'll work within a multidisciplinary team delivering person‐centred care.
