- Deliver comprehensive case management and intensive, transitional support to young people, single adults, and families experiencing homelessness.
- Conduct holistic, strength-based assessments to develop tailored, goal-oriented plans focusing on long-term housing stability and independent living.
- Provide expert care coordination, service navigation, and referral to connect clients with mainstream and specialist services (health, mental health, AOD, etc.).
- Offer targeted, flexible, and rapidly responsive support to clients with increased vulnerabilities and complex needs (e.g., trauma, multiple health issues).
- Work collaboratively with other agencies, participate in community networking, and convene multi-disciplinary team meetings to ensure integrated client support.

- Valid Victorian Driver’s Licence |Working with Children Check |Police Check
- Minimum requirement of Diploma of Community Services or other relevant or appropriate tertiary qualifications (refer to Position Description).
