Adaptive Home Health is seeking a Licensed Vocational Nurse (LVN) — Field Clinician to deliver skilled nursing visits in patients' homes across Texas. You will document thoroughly in HCHB, escalate changes to the RN/clinical team, and coordinate with scheduling to keep visits on track.

This full time role offers a productivity-based pay model with a per-point rate, plus a comprehensive benefits package. Flex scheduling supports field clinicians while maintaining patient-centered care goals.
