Uniting NSW.ACT is seeking a dedicated Case Manager to lead the delivery of person-centred home and community care services in Tamworth. You will coordinate care planning, act as the central client contact, and ensure services align with funding and client needs.

This full time, fixed-term role runs until 11 April 2027 and requires travel between Uniting sites and client locations, strong collaboration, and a commitment to quality and safety in care delivery.
