CCH Group is seeking a motivated Care Coordinator to organise and deliver high-quality in-home care across the community.
You will manage care workers, review care plans, and coordinate Rosters to ensure timely, compassionate support for clients.
A valid driving licence and access to your own vehicle are required due to travel between locations.
The role offers Monday–Friday, 9am–5pm with on-call duties on a rota basis, and the chance to grow within a trusted national care provider that values
