Darling Downs Health is seeking a Coordinator Home Care to help ageing clients remain independent at home.
You will coordinate personalised support, build partnerships with carers and health professionals, and lead home care staff to deliver secure, high-quality services.
You will drive service improvements and ensure compliance with aged care standards while supporting clients to achieve their goals and maintain dignity and independence in their home workplace.
