WA Country Health Service is hiring for a role that supports the delivery of remote aged care services by collaborating with communities, remote care workers and multidisciplinary teams to identify and meet client needs in line with KACS requirements.

The position requires willingness to travel to remote Kimberley communities, including overnight stays, and to work effectively in geographically isolated environments.
