NSW Health’s Hospital in the Home (HiTH) Intake team at Hunter New England Health seeks a part time Intake Registered Nurse at John Hunter Hospital to triage referrals and determine care needs for patients suitable for home-based care.

You will work with HiTH and hospital teams to coordinate safe, seamless care, support patients through transitions, and stay connected through daily huddles and accurate documentation.
