Australian Unity is seeking a Care Partner to support customers throughout their care journey, helping them live safely and independently at home or in a place of their choosing. You will work closely with customers, families, clinical teams and external providers to develop holistic care plans and coordinate services.

This permanent full-time, hybrid role offers flexible schedule and an hourly rate up to $45.56 plus super.
