As the Triage Officer, you'll be the first point of contact for Commonwealth Home Support Programme (CHSP) Aged Care clients, ensuring they are connected with the right allied health therapy services. This is a casual, work-from-home position offering flexible hours with the opportunity to work 2 to 4 days per week.

Key responsibilities

- Serve as the first point of contact for CHSP clients, managing inbound referrals and inquiries with empathy and professionalism
- Conduct initial clinical assessments, which inform therapy planning and care coordination by our clinical team
- Assess and document client medical history, falls history, risk factors, living arrangements, and other key intake data required for therapy sessions
- Perform detailed data entry and client record management using clinical software (Splose)
- Update and maintain internal database to manage client flow and follow-up requirements
- Coordinate with allied health professionals, clinical staff, and management to ensure timely and effective client care
- Communicate through internal systems including Microsoft Outlook and Teams for seamless coordination
- Uphold strict standards of privacy, confidentiality, and documentation accuracy in all client interactions

About you

- Hold a tertiary qualification in healthcare as an Enrolled Nurse (EN) or Registered Nurse (RN),



allowing you to maintain your AHPRA registration while working in this role; or hold a Certificate III or IV in Allied Health Assistance; or are currently studying a tertiary degree in Physiotherapy, Occupational Therapy, Speech Pathology, or Exercise Physiology; or hold a tertiary qualification in an allied health discipline, such as Physiotherapy, Occupational Therapy, Speech Pathology, Dietetics, Podiatry, or Exercise Physiology
- Experienced in customer service, with a robust focus on client satisfaction
- Tech-savvy and detail-oriented, able to handle data entry and record-keeping with accuracy
- Familiar with healthcare systems like Splose, or a fast learner willing to adapt
- Comfortable multitasking in a fast-paced environment
- A clear communicator, able to engage with clients, therapists, and internal teams effectively
- Previous experience in healthcare, aged care, or allied health administration is a plus
