South Metropolitan Health Service in Western Australia is seeking a Pharmacist to join its hospital pharmacy team. You will review and dispense medications, conduct medication reconciliation and clinical reviews, and provide expert medication advice to clinicians and patients.

You will support discharge planning, promote secure use of medicines and participate in medication safety initiatives within a multidisciplinary setting, delivering high-quality, patient-centred care.
