You'll be part of a great, supportive team that values collaboration, culture, and patient outcomes.Key ResponsibilitiesSupport and educate patients in managing chronic conditions, and improving health behaviours through care planning and regular reviewsConduct health assessments and promote preventative careCollaborate with GPs and multidisciplinary teams, including referral coordinationManage administrative tasks related to CCM appointmentsCoach patients on sustainable health behaviour changes.Promote preventative healthcare initiatives across the practice, e.g. healthy heart checks,



women's health checks and diabetes checks.Work with the practice team to grow the CCM program.About YouTo be successful in this role, you will have:Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN).
