Position Overview

Synergeer Care provides quality home and community-based care to NDIS participants, inpatient and community mental health consumers and the elderly.

As a Residential Care Officer at Synergeer Care, you will provide individualised, person-centred care to clients with diverse support needs. You will play a vital role in promoting well-being, independence, and social inclusion in alignment with NDIS standards and organisational policies.

Position Responsibilities and Tasks
Conduct assessments of client needs and contribute to the development of individualised care and support plans that reflect NDIS objectives and personal preferences.
Deliver personal care and daily living assistance, including support with hygiene, mobility, nutrition, and household tasks, to promote wellbeing and independence.
Support clients in accessing healthcare services, community participation opportunities, and structured social programs to encourage inclusion and personal development.




Apply behavioural and emotional support strategies, providing appropriate responses in situations involving mental health challenges or crisis intervention.
Maintain accurate and comprehensive records, case notes, and progress reports in line with organisational policies and NDIS quality and safeguarding standards.
Work collaboratively with families, allied health professionals, and community service providers to ensure coordinated and holistic service delivery.
Contribute to the orientation, training, and guidance of new staff, promoting secure practices and a consistent, person-centred approach to care.

Qualifications and Skills

Salary

The role offers a remuneration package ranging from $79,000 - $90,000, plus superannuation for a 38-hour work week.
