Wintringham Limited in Williamstown, VIC, is seeking an experienced Care Evaluator to join our Not-for-Profit team.
You will provide clinical guidance, care planning and hands-on support to residents, working closely with care staff to promote independence, dignity and quality of life.
Reporting to the Residential Site Manager, you will monitor changes in health and medications, develop and review Support Plans, and ensure high standards of person-centred care across shifts.
