The Team Leader, Assessment and Planning is pivotal in the delivery of quality carer services and plays an essential role in the leadership team of Carer Supports.
The Team Leader, Assessment and Planning provides strong leadership to oversee assessment, goal planning, service coordination and referrals for carers receiving supports through the Carer Services Supports Program.
The role directly supervises a team of Assessment and Planning Officers and Senior Assessment and Planning Officers, who in return have operational line management.
YOU WILL BE RESPONSIBLE FOR:
Reporting to the Carer Supports Lead, this role:
Provides clinical and line management to a team of Assessment and Planning Officers and the Senior Assessment and Planning Officers.
Provides indirect line management to an assigned team
Ensures the team meets targets, KPIs and funding guidelines.
Works closely with the Senior Assessment and Planning Officers to ensure the team runs efficiently and best practice is adhered to for assessments, goal planning, service coordination and referrals.
Acts as an escalation point for the team
Provides clinical supervision and clinical consults for the team as well as for other teams as required and undertake urgent clinical intervention and triage.
As member of the leadership team, actively contributes to the management, strategic planning and advancement of the Carer Supports Program.
Undertakes and contributes to program-wide reporting and data collation tasks as directed.
Works closely with the other Team Leader, Assessment and Planning, to enable consistent and effective work practices.
Ensures feedback and incidents are managed in line with Holstep Health's policies and procedures.
Facilitates, receives, manages and incorporates feedback from carers and stakeholders into quality improvement activities.
Contributes to Carer Gateway-wide initiatives to improve and streamline practice and processes.
Develops and maintains positive relationships with key stakeholders, including relevant government services, service providers and key referrers, including internal colleagues.
Demonstrates leadership by ensuring compliance with the Scope of Practice.
Other Duties
Support staff in the implementation of the Program and Holstep Health's Strategic Plan.
People management through the employment life cycle, including recruitment, orientation, performance management, development, leave, and separations.
Be responsible for team outcomes, ensuring targets and funding requirements are met, and good external relationships are maintained.
Ensure compliance with all relevant legislation, funding guidelines, service standards and contractual obligations.
WHAT YOU NEED TO SUCCEED:
Bachelor qualification in social work, psychology, nursing, allied health or appropriate equivalent discipline.
Minimum of 5 years' experience in similar role.
Demonstrated ability to lead high-performing, multi-disciplinary teams.
Sound knowledge of the community care sector.
Proven ability to foster and maintain relationships with internal and external stakeholders and to relate to and work effectively with a diverse range of individuals and communities.
Excellent organisational, problem-solving, interpersonal and communication skills
Sound financial management, budgeting, reporting and compliance skills.
Proficiency in the use of information and communication technologies.
