Mounties Care Home is seeking a dedicated Care Partner for a part-time role (3 days per week) at our Dandenong office. You will coordinate care and services to help clients stay independent at home and in the community, while guiding a team of carers to ensure quality services.
Key duties include developing care plans, budgeting, assisting access to My Aged Care, and community visits. Robust communication, confidentiality, and to work effectively with families and providers are essential.
