Job Description

Responsible for recruitment, information and referral to resources for ex-offenders. Maintaining contact with clients in the community in an effort to offer support and guidance and linking clients with needed services to promote employment, treatment, and housing.

Scope of Duties:
n
n
Recruit clients to participate in program including visiting the county jail to interview prospective clients.
n
Complete intake interviews with clients and secure program participation agreements.
n
Assist clients apply for social entitlements such as GA, TANF, etc. when appropriate.
n
Link homeless clients to housing.
n
Monitor and document client progress.
n
Assess client needs and work with client to develop goals.
n
Update service plan with client input.
n
Provide motivational support to clients in program and maintain support throughout participation.
n
Schedule and facilitate meetings with clients as required.
n
Identify and/or develop resources and link clients to needed services.
n
Facilitate scheduled
workshops/programs/groups
addressing client's needs.
n
Develop appropriate discharge plan on all assigned clients.
n
Participate in required internal/external meetings.
n
Complete all required reports, data collection and other documentation.
n
Interact with all clients, staff, and outside contacts with respect and courtesy.
n
Maintain client and staff confidentiality in compliance with administrative policies and procedures.
n
Attend and participate in regularly scheduled staff meetings, supervisory conferences, and training sessions.
n
Other duties as assigned.

Qualifications Education
Required
n
Bachelors or better in Human Services or related field.
Experience
Required
n
Bachelor's Degree preferably in a Human Services related field or at least 5 years relevant experience. Transferable experience is preferred. Must possess effective written and verbal communication skills, and an ability to interact appropriately with clientele. Basic computer skills are preferred. Must pass a criminal background check.
