We're looking for an organised, proactive and resilient
Services Coordinator – CHSP
to join our passionate team and help coordinate the delivery of our Commonwealth Home Support Programme services.
n
This is a
fast-paced, high-volume coordination role
. You'll manage enquiries and referrals, complete assessments and service documentation, coordinate service commencement and reviews, and monitor services to ensure they remain aligned with each client's
choices, goals and changing needs
.
n
You'll be part of a team that genuinely cares about the people we support and works together to make sure available services reach older people who need them.
About the Role n
As our
Services Coordinator – CHSP
, you'll coordinate services from enquiry and referral through to commencement, review and ongoing service delivery.
n
Working closely with clients, families, My Aged Care, referral partners and our internal teams, you'll help ensure services are delivered efficiently, accurately and with the person at the centre.
n
Your responsibilities will include:
n

Manage CHSP enquiries and referrals from initial contact through to service commencement.
n
Complete assessments, Support Plans, reviews and required service documentation.
n
Coordinate services that reflect each client's choices, goals and changing needs.
n
Monitor service utilisation and delivery, proactively following up gaps and opportunities for additional support.
n
Contribute to achieving contracted CHSP program outputs, helping ensure funded services reach people who need them.
n




Maintain accurate records to support DEX reporting, quality, compliance and program performance.
n
Build referral relationships and represent FOCUS Connect at community meetings, networks, expos and events.
n
Ensure services are delivered in line with CHSP Program Guidelines, Aged Care Quality Standards and the FOCUS Connect Philosophy of Care.
