The
Qualified Support Worker
works as part of the Pearl Home Care Gladstone care team to provide high-quality, person-centred support to consumers and participants in their homes and within the community.
n
Working under the direction of the
Care Manager and Resource Coordinator
, the Support Worker is responsible for delivering services in accordance with each consumer or participant's individualised care plan, preferences, goals and assessed needs.
n
The successful applicant will demonstrate compassion, professionalism, reliability and a genuine commitment to supporting older people and people with disability to maintain their independence, dignity, choice and quality of life.

Qualified Support Worker

Key Responsibilities n
The Qualified Support Worker will:
n
n
Read and understand consumer and participant care plans and deliver services in accordance with the documented care requirements.
n
Provide personal care and daily living assistance in a respectful, caring and professional manner.
n
Recognise when a consumer or participant's needs,
abilities or condition have changed from what is documented in their care plan and promptly report this to the Care Manager or Coordinator.
n
Escalate concerns regarding changes in a consumer or participant's health, wellbeing, behaviour, safety or general condition.
n
Maintain the privacy, dignity and confidentiality of consumers and participants at all times.
n
Complete all required documentation, including progress and shift notes, accurately and within the required timeframe.
n
Provide domestic assistance and maintain agreed areas of the consumer or participant's home to a high standard, including bathrooms, kitchens, living areas and other designated areas.
n
Assist consumers and participants with shopping while following their individual preferences, instructions and available budget.
n
Safely transport consumers and participants to appointments, social activities, shopping and community outings while supporting his comfort, dignity and wellbeing.
n
Prepare meals in accordance with individual dietary requirements, preferences and documented instructions.
n
Encourage independence and support consumers and participants to remain actively involved in decisions relating to their care and daily activities.
n
Follow all organisational policies, procedures, infection-control requirements, work health and safety requirements and safe work practices.
n
Report incidents, hazards, near misses and concerns promptly in accordance with organisational procedures.
n
Work collaboratively with Care Partners, Coordinators,
clinical staff and other members of the care team to ensure continuity and quality of care.
n
About You n
To be considered for this role, applicants must have:
n
n
Successful completion of
Certificate III in Individual Support (Ageing and/or Disability), Certificate III in Aged Care
, or another recognised qualification suitable for providing home care services.
n
A satisfactory professional reference from a current or recent employer demonstrating competence, reliability and confidence in performing support work.
n
A
National Police Check
that meets current organisational and legislative requirements.
n
A current
First Aid Certificate
.
n
Current
CPR certification
.
n
Current
Manual Handling training
.
n
A current
Working with Children Check (Blue Card)
where required for the role.
n
A current
NDIS Worker Screening Clearance (Yellow Card)
where required for the role.
n
Evidence of the
right to work in Australia
.
n
A current unrestricted or appropriate
Australian Driver Licence
.
n
Access to a
reliable, registered and appropriately insured vehicle
for work-related travel.
n
