McArthur is currently seeking an experienced Social Worker for an immediate contract opportunity within a hospital environment in Wollongong.
n
This is an initial 3-month assignment with the potential to extend. The role can accommodate 3-5 days per week (part time or full time), with full-time availability preferred.
The chance n
Working as part of a multidisciplinary healthcare team, you will provide social work support to patients and their families across a range of presentations and circumstances.
n
Your responsibilities may include:
n
n
Undertaking psychosocial assessments and providing appropriate social work interventions
n
Supporting patients and families through complex health and personal circumstances
n
Contributing to discharge planning and coordination of ongoing supports
n
Working collaboratively with medical, nursing and allied health professionals
n
Connecting patients and families with appropriate community and support services
n
Supporting patients with aged care and/or palliative care needs
n
Maintaining accurate clinical documentation and case records
n
About you n
To be considered, you will ideally have:




n
n
A recognised qualification in Social Work
n
Previous Social Work experience within a hospital or healthcare environment
n
Experience working with aged care and/or palliative care patients would be highly regarded
n
Strong psychosocial assessment and case management skills
n
Experience working within multidisciplinary teams
n
Excellent communication and interpersonal skills
n
Ability to work independently in a busy clinical environment
n
Current or eligible membership with the Australian Association of Social Workers (AASW)
n
Ability to satisfy relevant pre-employment and compliance requirements, including a National Police Check
n
