Practice Information
This privately owned mixed billing medical practice is the only GP practice in town and has been serving the local community for more than nine years.
The practice is supported by two permanent doctors on a fly-in fly-out roster, an experienced Practice Manager, nursing team and reception staff.
Children under 16 years of age and concession card holders are bulk billed.
Position Description
The Registered Nurse - Chronic Disease Management will support the delivery of high-quality, patient-centred care through chronic disease management, preventative health programs, care coordination and population health activities.
The role involves working collaboratively with General Practitioners and the broader healthcare team to improve health outcomes for patients with chronic and complex conditions.
Responsibilities include care planning, health assessments, patient education, recall and reminder systems,
health promotion activities and supporting quality improvement initiatives.
Position Requirements
Current registration as a Registered Nurse with AHPRA
Experience in chronic disease management or primary healthcare highly regarded
Strong communication and organisational skills
Ability to work effectively within a multidisciplinary team
Commitment to delivering patient-centred care
Valid authority to work in Australia
