In this role, you will coordinate flexible services, developcareplans in partnership with clients, and lead a team of community support workers to delivertimelyandappropriate care.
You will work in a collaborative environment, building strong relationships with clients, families, and service providers, while also developing your skills incarecoordination, leadership, and continuous service improvement within a consumer-directedcaremodel.

WHO ARE WE LOOKING FOR?
Do you have experience coordinating client services in agedcareor a community setting,



with a strong focus on person-centred or consumer-directed care?
Are you confident conducting home-based assessments, developingcareplans, and prioritising referrals to meet client needs effectively?
Can you supervise, support, and motivate community-based staff while managing rosters and responding to changing client requirements?
Do you havestrong communicationand interpersonal skills to build relationships with clients, families, and a range of stakeholders from diverse backgrounds?
Are you highly organised with the ability to manage competing priorities,maintainaccuraterecords, and ensure compliance with relevant guidelines and reporting requirements?

HOW WILL YOU CONTRIBUTE?
Deliver high-quality, person-centredcarecoordination that supports older people to remain independent in their own homes
Undertake assessments, developcareplans, and manage referrals to ensuretimelyandappropriate servicedelivery
Lead and support community support workers, ensuring effective rostering, communication, and qualitycareoutcomes
Build strong relationships with clients, families, and service providers to ensure the rightcareis delivered at the right time
Maintainaccuratedocumentation, manage data, and contribute to continuous improvement and compliance activities
