We deliver practical, person-centred services that strengthen community wellbeing and belonging.About the role:Provide home care services aligned to Out of Hospital Care goals, including domestic assistance, personal care, support services that assist recovery and independence.Assist participants during the end-of-life stages of care.Apply duty of care and secure work practices in client homes, and identify, document, and elevate OHC escalation triggers promptly.For a copy of the position description, please visit our website: communityservices1.org/careers/About you:Certificate III in Individual Support (Ageing) or equivalent, relevant experience delivering home care/aged care support (domestic assistance and/or personal care)



and 3+ months aged carer experience.Experience supporting clients with diverse cultural, emotional and health needs in community or out-of-hospital settings.Ability to follow care plans and monitor client wellbeing.What we offer:Casual role.
Remuneration aligned to SCHADS Award - Home Care Employee Level 2 and 3 (casual), plus super.
