You will coordinate care for clients with mental health issues, working with a multidisciplinary team to deliver acute care and recovery-focused support across the district.
The role emphasizes person-centred care, collaboration with families and GPs, and aim to enable informed health decisions.
Full time permanent position with supportive supervision and career development.
