Job Description
BlueCare, part of UnitingCare, seeks a Care Partner to deliver person-centred care, oversee care planning, and coordinate services in community settings. You will work closely with clinical teams to ensure optimal outcomes for clients funded through aged care services.

The role requires driving to client sites, travel across Redland Bay to surrounding areas, and robust communication skills to support diverse client needs. A Certificate IV and a current driver's licence are essential.
