The role focuses on comprehensive assessments across community and hospital settings to inform client-centred care plans within a wellness framework.

Applicants from Senior Occupational Therapist, Senior Physiotherapist, Senior Social Worker or Clinical Nurse backgrounds are encouraged to apply, and you will work closely
