About the opportunity

In this role, you will:
- Triage referrals and identify patients suitable for care at home.
- Use your clinical expertise to review patient information and determine care needs.
- Work collaboratively with HiTH and hospital teams to coordinate safe, seamless care.
- Support patients and families through their transition from hospital to home.
- Stay connected with the team through daily huddles, sharing updates and discussing patient needs.
- Keep care moving by maintaining accurate documentation and following up referrals.

What we're looking for
We’re looking for a Registered Nurse who is:
- Registered as a Nurse with AHPRA and holds a relevant nursing qualification.
- Confident in clinical reasoning, judgement and triage.
- A strong communicator who works well with patients, families and healthcare teams.
- Able to work autonomously while being a supportive team member.
- Organised, adaptable and able to manage competing priorities.
- Proactive, resilient and committed to patient-centred care.
- Confident coordinating care and working with multiple stakeholders.
Experience in Hospital in the Home, community or acute care is highly regarded.
Knowledge of HiTH referral processes, eligibility criteria and escalation pathways is desirable, but can be supported through orientation and on-the-job learning.
