Health Workforce in Middlemount, Central Queensland, is seeking a Registered Nurse for Chronic Disease Management to support patient-centred care in a rural general practice. The role focuses on chronic disease management, preventative health programs, care coordination and population health activities.

Accommodation is provided. The contract runs from 12 October 2026 to 30 June 2027, with full time or part-time arrangements and a negotiable salary based on experience and qualifications.
