IPC Health is seeking an experienced Strengthening and Restoring Family Services Practitioner to support families across Brimbank, Melton and Western Melbourne. Based at the Hoppers Crossing campus, you will deliver assertive, relational case management within Strengthening Families and Restoring Families streams.

You will work with multidisciplinary teams to assess needs, develop SMART care plans, strengthen parenting capacity and connect families with supports to promote safety and long-term
