Amplar Health, part of the Medibank Group, is seeking a Care Integration Nurse to deliver patient-centred nursing care in patients' homes under the Amplar Health Home Hospital model. You will collaborate with Medical Officers, Nurse Care Coordinators and service provider networks to perform clinical reviews within a multidisciplinary team.
The role involves community visits, shift rosters on a 7-day cycle, and using a company vehicle starting from our Eastwood office.
