Wellington Correctional Centre seeks a Case Management Officer to deliver evidence-based case management to a mixed offender caseload. You will assess risks and needs, develop personalised plans, and collaborate with internal and external service providers to support rehabilitation.
You will work within the Service Integration team to monitor progress, prepare reports, and build relationships with stakeholders to aid reintegration and reduce reoffending.
