The Role: managing a community-based caseload, primarily supporting Aged Care clients (small proportion of NDIS participants)

Your responsibilities will include:

- Functional assessments within clients' homes
- Assistive technology prescription and equipment recommendations
- Minor and major home modification assessments
- Developing intervention plans to maximise independence and safety at home
- Reviewing client progress and completing high-quality clinical reports
- Working collaboratively within a multidisciplinary team

You'll need:

- Current AHPRA registration as an Occupational Therapist
- Current driver's licence and access to a vehicle
- Strong communication and time management skills
- A passion for delivering person-centred care and achieving outstanding outcomes for clients
