With substantial chronic disease management work and a dedicated CDM Nurse, you'll have support to develop care plans, manage complex needs and build lasting patient relationships. Full-time nursing and allied health support bring additional expertise into the practice, helping you coordinate care around your patients.

The Practice Manager actively supports care plans and health assessments, bringing practical involvement to an exceptionally well-run clinic.

Requirements:

- Vocational Registration - FRACGP, FACRRM or equivalent
