St Vincent's Care at Home is recruiting a Registered Nurse to deliver acute-equivalent care in the community across Toowoomba. You will work with MCAH leads, VMOs and a range of professionals to provide tailored, coordinated care for patients receiving treatment at home.

You will assess needs, develop care plans, administer interventions such as IV therapies and wound care, and coordinate protected transitions while aiming to reduce hospital admissions and improve patient outcomes in a home-based
