Community Nursing | Clinical Governance | Medication Management | Training
Aspire Hub is seeking an experienced Registered Nurse to support safe, high-quality clinical practice across our NDIS and Child Safety services.
This varied role combines community nursing, medication management, employee training, clinical audits and continuous improvement. You will work closely with operational teams and frontline employees to ensure Participants and Young People receive safe, consistent and individualised support.
About the Role
You will provide clinical expertise across our services while strengthening the capability of our frontline workforce and supporting effective clinical governance.
Key Responsibilities
Community Nursing and Clinical Support
- Provide community nursing, clinical assessments and advice
- Support Participants and Young People with complex health needs
- Liaise with GPs, specialists, pharmacies and allied health professionals
- Develop and review individual health and clinical management plans
- Provide clinical guidance following health incidents, hospital admissions or changes in health needs
Medication Management and Clinical Governance
- Oversee medication management systems and documentation
- Maintain medication information and data within our CRM
- Conduct internal medication audits across houses and systems
- Identify and address medication risks, documentation gaps and areas for improvement
- Review medication incidents and support corrective actions
- Ensure medication records, charts and authorities remain current and accurate
Training and Workforce Capability
- Deliver medication training to all new support employees and annual refresher training
- Conduct practical competency assessments and provide coaching where required
- Deliver participant-specific clinical training, including areas such as subcutaneous injections and other health procedures
- Develop training resources and clinical guidance materials
- Provide additional training and coaching where audits or incidents identify practice gaps
- Deliver medical training to external providers when requested
Management Plans, Quality and Compliance
- Develop, review and implement health and clinical management plans
- Support plans relating to diabetes, epilepsy, bowel and urinary care, respiratory conditions, enteral feeding, dysphagia, wound care and other complex health needs
- Assist with the development and review of clinical policies and procedures
- Contribute to internal audits, quality reviews and external audit preparation
- Review clinical incidents and audit findings to identify trends and improvement opportunities
- Work collaboratively with Operations and People And Culture to identify emerging clinical and training needs
About You
You are a Registered Nurse who is comfortable working across clinical care, training, auditing and clinical governance.
You will bring:
- Current AHPRA registration as a Registered Nurse
- Strong knowledge of medication management and medication safety
- Confidence completing clinical assessments, audits and competency assessments
- Strong clinical documentation and record-keeping skills
- Excellent communication and stakeholder engagement skills
- Strong attention to detail and a commitment to safe, person-centred practice.
Experience in community nursing, disability, complex care, Child Safety or residential services is highly regarded, particularly experience delivering medication or clinical training and developing health management plans.
