We're looking for a passionate Mental Health Clinician to join our MH Connext team and help people living with severe and complex mental health challenges achieve meaningful recovery outcomes.
Description: About the Role
As a Mental Health Clinician, you will work within our MH Connext community outreach service, providing person-centred, recovery-oriented mental health support to people living with severe and complex mental health concerns, including those experiencing co-occurring substance use challenges.
This is a highly autonomous and rewarding role where you will build therapeutic relationships, coordinate care, undertake assessments, and connect people with the supports they need to thrive in their communities.
While we welcome applications from a range of mental health professionals, Registered Nurses with mental health experience will be highly regarded for this position.
What You'll Be Doing
- Conduct comprehensive mental health, psychosocial, risk and safety assessments.
- Manage an allocated caseload and coordinate recovery-focused support.
- Develop, implement and review individual recovery plans with consumers and their support networks.
- Deliver evidence-based therapeutic and psychosocial interventions within your professional scope.
- Provide crisis intervention, de-escalation support and safety planning.
- Connect consumers with GPs, mental health services, allied health providers and community supports.
- Facilitate psychoeducation and therapeutic groups where required.
- Work collaboratively with families, carers, multidisciplinary colleagues and external stakeholders.
- Maintain accurate, compliant and timely clinical documentation.
About The Service
MH Connext is a community-based mental health outreach service that provides person-centred, recovery-oriented support to people experiencing severe and complex mental health challenges, including those who may also experience co-occurring substance use concerns. The service works alongside consumers to improve their mental health, wellbeing, independence and connection to community. Clinicians provide assessment, care coordination, recovery planning and support, while helping people access the services and resources they need to achieve their goals.
MH Connext takes a holistic approach to care, recognising the importance of mental, physical, social,
cultural and spiritual wellbeing in recovery.
Skills and Experiences: About You
Essential
What you'll bring
- Relevant qualification and qualified registration in one of the following:
- Registered Nurse (AHPRA)
- Occupational Therapist (AHPRA)
- Social Worker (AASW eligible)
- Experience working in mental health services, ideally within community-based settings.
- Strong assessment, risk management and care planning skills.
- Experience delivering therapeutic or psychosocial interventions.
- Understanding of recovery-oriented, person-centred and trauma-informed practice.
- Ability to build strong relationships with consumers, families, carers and stakeholders.
- Excellent communication, problem-solving and teamwork capabilities.
Highly regarded
- Registered Nurses with demonstrated mental health experience.
- Postgraduate qualifications in Mental Health and/or Alcohol and Other Drugs.
- Experience working with people experiencing complex mental health and co-occurring substance use concerns.
- Experience facilitating groups and psychoeducation programs.
This role is fixed term full time until 30 June 2027, working Monday to Friday.
