Support hospital to community transitions through complex wound care, medication and palliative care support in Tamworth homes and clinics.

Where you’ll be working

You’ll be based at Tamworth Community Health Service, supporting seamless patient transitions from Tamworth Hospital into the community, plus working across community clinics and home visits. This dynamic setting lets you connect directly with patients where they feel most comfortable. Together, this creates rewarding opportunities to make a real difference in regional healthcare delivery.

About The Role

As a Registered Nurse – Community Health, you’ll manage an individual caseload on a 7-day rotating roster (including public holidays), focusing on hospital-to-community discharge support alongside community clinic work.

Your responsibilities include complex wound care, medication management, palliative care support, catheter management, general nursing assessments and clinical referrals, working both independently and collaboratively to deliver best-practice, compassionate care. Reporting to community health leadership, this varied role lets you build strong patient relationships while coordinating seamless care across settings.

About You

You’re a skilled community nurse with recent experience in complex care needs and a genuine commitment to patient-centred support in dynamic home environments. You bring confidence, sensitivity and strong decision-making to collaborate effectively with patients, families and teams.

You Bring

- Current AHPRA Authority to Practice as Registered Nurse
- Current unrestricted NSW driver’s licence with fitness and willingness to travel
- Recent experience in complex wound care, catheter management, and medication administration




- Confidence and sensitivity supporting palliative care patients and families
- Strong time management and decision-making skills
- Commitment to patient-centred care and collaborative teamwork, with adaptability in community settings

These skills and qualities will help you excel in this rapid-paced community role, with initial buddying, CHIME/SDPR training, and ongoing clinical support from Nurse Practitioners (NPs), Clinical Nurse Consultants (CNCs), Clinical Nurse Specialists (CNSs) and online platforms.
