As the organisation continues to grow, they are seeking a Care Coordinator based in Toowoomba to support the continued delivery of high quality care, support frontline teams and ensure participants receive safe, consistent and individualised support. About the Role and Your Responsibilities Reporting to the Regional Leader, you will take ownership in coordinating and overseeing supports for NDIS participants with complex disabilities, mental health needs, behaviours of concern and high risk support requirements. This is a hands on coordination role where you will manage participant care plans, risk assessments, incidents and documentation while working closely with families, guardians, Support Coordinators, Behaviour Support Practitioners, clinicians and other external stakeholders. You will also provide guidance and support to frontline staff, helping them confidently deliver complex care while maintaining high standards of safety, compliance and participant centred practice.
Some of your responsibilities will include: Deliver proactive case management and care coordination tailored to each participant’s individual goals, risks and support needs Coordinate supports across families, guardians, Support Coordinators, allied health professionals and other providers Develop, review and maintain individualised participant care plans Manage complex and high risk incidents, including follow up, root cause analysis and debriefing Ensure restrictive practices are appropriately authorised, reviewed and compliant with NDIS Quality and Safeguards Commission requirements. Oversee the development, implementation and ongoing monitoring of Positive Behaviour Support Plans (PBSPs). Prepare accurate and detailed reports and documentation for internal and external stakeholders, including statutory agencies. Coach and supervise Support Workers, providing training, guidance, debriefing and reflective practice to support quality service delivery. Participate in the paid rotating on-call roster and respond to after-hours calls within required response time. Liaise with legal guardians, clinicians, Behaviour Support Practitioners and the NDIS Commission to coordinate effective wrap around supports for participants. Skills & Qualifications Previous experience in Care Coordination, Case Management or a similar role within the NDIS, Disability or Community Services Sector Tertiary qualifications in Social Work, Psychology, Disability or a related field will be highly regarded. Demonstrated experience supporting people with complex Disabilities, Mental Health or Behavioural needs,
with Support Worker or lived experience highly desirable Strong understanding of NDIS Practice Standards, Quality and Safeguards requirements, restrictive practices and reportable incidents Proven ability to lead, coordinate and support frontline teams, including delegation, coaching and supervision Strong care planning, assessment, risk management, documentation and compliance skills Excellent organisational and time management skills, with the ability to prioritise competing responsibilities and work autonomously Ability to remain calm, adaptable and solutions focused when managing high pressure, complex or crisis situations Robust stakeholder engagement skills with the ability to build effective relationships with participants, families, clinicians and external providers Excellent written and verbal communication skills, including professional report writing and accurate documentation Self motivated with strong attention to detail, problem solving skills and a drive to take ownership and achieve positive outcomes Current Blue Card and NDIS Worker Screening Check, or willingness to obtain Full working rights in Australia Benefits Salary of $95k to $100k plus super Join a growing, purpose-driven organisation making a positive impact in the community Opportunity to develop your leadership skills through coaching and supporting frontline teams Supportive leadership team that values collaboration and professional development Access to professional development pathways and expert led training How to Apply: If you are a motivated and experienced professional looking for your next opportunity in the NDIS sector and are passionate about delivering quality outcomes for people with complex support needs, we’d love to hear from you.
