Clinical Care Partner / Full Time / South Yarra
Absolute Care & Health (Absolute) works to revolutionise the way in care and support is provided to people in their home. We believe people are happiest in their home and we strive to assist people to stay in their home by providing the support and care they need to live as independently as possible. The Disability Support worker primarily supports Absolutes NDIS Participants to achieve their goals as set out within their NDIS plan. This also includes the provision of personal care, companion care, and community access.
The primary purpose of this role is to deliver high-quality clinical care management that
supports clients to live safely, independently and in accordance with their goals while receiving services under the Support at Home program.
The Clinical Care Partner is responsible for assessing clients' clinical needs, developing and reviewing the clinical components of client care plans, monitoring health and wellbeing, andcoordinating clinical supports that promote wellness, reablement and client choice. The role applies clinical knowledge and qualified judgement to identify changes in client needs, respond to clinical risks and ensure care remains safe, responsive and person-centred.
Working as part of an Integrated Care Team, the Clinical Care Partner collaborates closely with the Anchor Registered Nurse and Non-Clinical Care Partner to deliver coordinated, person-centred care management. The role also works in partnership with clients, their representatives, healthcare professionals and external providers to ensure services are coordinated, clinically appropriate and aligned with client goals, preferences and available funding.
Working within a friendly, close-knit team environment, this varied role will include:
• Undertaking clinical assessments and ongoing clinical care management.
• Developing, implementing and reviewing the clinical components of client care plans.
• Monitoring client wellbeing and responding to changes in clinical needs.
• Applying wellness and reablement principles to support client independence.
• Coordinating clinical supports and liaising with healthcare professionals and external
providers.
• Collaborating with the Anchor Registered Nurse and Non-Clinical Care Partner to deliver
coordinated, person-centred care.
• Escalating complex clinical matters to the Anchor Registered Nurse.
• Maintaining accurate clinical documentation and supporting continuous quality
improvement.

assessment, care planning and ongoing clinical review.

practice and compliance with legislative and regulatory requirements.

Duties and Responsibilities
Operational
➢ Build and maintain positive, professional relationships with clients, their representatives
and key stakeholders to support person-centred care management.
➢ Deliver coordinated clinical care management as part of the Integrated Care Team,
ensuring services remain responsive to clients' goals, changing needs and available
funding.
➢ Work collaboratively with the Anchor Registered Nurse and Non-Clinical Care Partner to
provide coordinated, integrated care management and a seamless client experience.
➢ Develop, implement and review the clinical components of client care plans in
partnership with clients, their representatives and the Integrated Care Team.
➢ Monitor client wellbeing and identify changes requiring clinical review, intervention or
escalation.
➢ Work collaboratively with the Integrated Care Team to support effective budget utilisation
and sustainable service delivery.
➢ Maintain accurate, timely and compliant documentation in accordance with organisational
policies, legislative requirements and professional standards.
➢ Promote continuous improvement through professional practice, innovation and
participation in quality improvement initiatives.
Clinical
➢ Undertake clinical assessments in accordance with organisational policies, professional
standards and the Support at Home Program.
➢ Apply clinical knowledge and professional judgement to assess client needs, identify
risks and determine appropriate clinical interventions.
➢ Develop, implement and review the clinical components of client care plans that
promote wellness, reablement, client choice and independence.
➢ Monitor client health and wellbeing and respond appropriately to changes in clinical
condition.
➢ Undertake clinical reviews and risk reviews where indicated to ensure care remains
appropriate, safe and responsive.
➢ Coordinate clinical interventions and liaise with General Practitioners, Allied Health
Professionals, Specialists and other healthcare providers to support integrated care.
➢ Escalate complex clinical matters, significant clinical risks and high-complexity clients
to the Anchor Registered Nurse in accordance with organisational policy.
➢ Approve client care plans where delegated in accordance with organisational policy.
➢ Maintain skilled registration, clinical competence and evidence-based practice in
accordance with legislative, regulatory and professional requirements.
