We are working with a small provider who is looking for a Clinical Care Coordinator who is a hands-on clinical leader and enjoys working in change environments and takes pride in achieving measurable improvements.
The facility has experienced significant challenges in recent years and is currently working through a formal improvement process. Since transitioning to a recent leadership team, substantial positive changes have already been implemented, including refreshed team, enhanced clinical oversight, and investment in quality systems.
As the Clinical Care Coordinator, you will play a critical role in supporting the home's ongoing improvement journey, partnering closely with the Residential Manager, Care Manager and wider leadership team to drive quality and risk management initiative, continuous improvement activities, staff education and development and clinical documentation and auditing processes.
This role would suit a clinician who enjoys rolling up their sleeves, leading from the front,
mentoring teams through change, and taking pride in seeing tangible improvements in performance and resident outcomes.
To be successful in this role, you will have:
- Current AHPRA registration as a Registered Nurse
- Previous experience as a Clinical Care Coordinator, or similar leadership position within aged care
- Strong knowledge of the Aged Care Quality Standards
- Experience managing compliance, auditing, and quality systems
- Excellent leadership and team development capabilities
- Strong communication and stakeholder engagement skills
- A proactive and solutions-focused approach
