About the Program

The Wesley care finder program provides intensive support to vulnerable older people who are unable to navigate the aged care system independently and do not have a trusted person available to assist them.

Clients may experience multiple and complex challenges, including social isolation, cognitive decline, poor health, financial hardship, housing instability and difficulty engaging with services. The program helps clients access My Aged Care and connect with appropriate aged care, health, housing and community supports

About the role

This role requires demonstrated casework experience supporting clients with complex and multiple needs. Using a person centred and strength based approach, the Case Worker will undertake assertive outreach within the community, build trust with older people who may be hesitant to engage, and provide persistent and respectful follow-up.

The role includes assessing needs and risks, developing agreed support strategies, assisting clients to navigate My Aged care, coordinating referrals and advocating across aged care, health , housing and community service systems. Working primarily in clients’ homes and community settings. The successful applicant will require strong professional judgement, clear boundaries, excellent documentation skills, and the ability to work independently in complex situations.

Regular travel within office hours is a significant part of this role

Essential skills/knowledge

- Diploma or higher in a relevant field (e.g., aged care, community services, social work, allied health, nursing)

- Proven experience supporting older people, people with disabilities, or individuals experiencing social disadvantage using person-centred, strengths based practice.‑centred, strengths‑based practice.

- Ability to identify client needs, provide appropriate service recommendations, and navigate multiple service systems.

- Demonstrated commitment to supporting people from diverse backgrounds, including Aboriginal and Torres Strait Islander peoples, Culturally and Linguistically Diverse communities, LGBTIQA+ communities, and rural/remote populations.

- Proven ability to build and maintain strong relationships across service providers, networks, and community organisations.

- Demonstrated ability to work both independently and collaboratively within a multidisciplinary team.

- Proficiency with Microsoft Office and experience using case management or client record systems for documentation and workflow management.

- Strong understanding of confidentiality obligations and privacy legislation relating to sensitive client information.

- Strong problem-solving skills, including the ability to assess issues, identify solutions, and take appropriate action within role boundaries.

- Demonstrated resilience, emotional maturity, and the ability to work effectively under pressure and manage competing priorities.

- Ability to remain calm, professional, and effective in challenging, distressing, or high-pressure situations‑pressure situations

Salary and employment conditions are in accordance with the Social, Community, Home Care and Disability Services Industry Award 2010, Level 5, with capacity for salary packaging opportunities and the provision of a fully maintained motor vehicle available for private use.
