Working alongside the Senior Hospital in the Home Pharmacist, you will provide high-quality clinical pharmacy services to patients receiving acute care within their own homes and residential care facilities.
This varied role offers the opportunity to work across hospital and community settings, supporting the safe, effective and appropriate use of medicines while working closely with patients, carers and multidisciplinary healthcare teams.
As part of the role, you will travel to patients' homes and residential facilities to provide clinical pharmacy services and support continuity of care outside the traditional hospital environment.Your responsibilities will include:Providing clinical pharmacy services to HITH and Residential Inreach patients in their homes and residential care facilitiesObtaining comprehensive and accurate medication histories and undertaking medication reconciliationReviewing medication therapy and providing recommendations to support safe and optimal medication useProviding medication counselling and education to patients, families and carersPromoting the safe, timely and cost-effective use of medicinesCollaborating closely with medical, nursing, pharmacy and other healthcare professionalsLiaising with community pharmacies, general practitioners and other external healthcare providers as requiredSupporting safe transitions of care between hospital, home and residential care settingsMaintaining accurate and timely clinical documentationSalary: Classifications for this position will range from SX2–SX5 ($94,******–$106,****** per annum, pro rata for part time).
Your ContributionYou are an experienced hospital Pharmacist with strong clinical judgement,
