Mounties Care Home care is seeking a dedicated Care Partner to join our team on a full time basis at our Hope Island, QLD office. You will coordinate care and services to help clients remain as independent as possible at home and in the community, with oversight of a team of Carers to ensure quality services are delivered.
The role involves developing client care plans, managing budgets, and engaging with clients, families and service providers to maintain high standards of care.
