Job Description
Monash Health is seeking a Senior Clinical Case Manager for Adult Mental Health on a full time basis based in Dandenong, VIC.
The role involves lead responsibilities in SECU Diversion, conducting outreach and clinic-based assessments, and coordinating care across medical, allied and nursing teams.
You will provide patient-centred, recovery-focused care, support individuals in their journey toward greater wellbeing, and work within a multidisciplinary team to deliver intensive outreach services
