Develop, implement and deliver primary health care within an integrated mental health service workplace with a multidisciplinary team focus including prevention, education, intervention and promotion activities and strategies to individuals, families and community groups.
The provisions of these services are to consumers and carers experiencing and affected by serious mental health disorders and other serious mental health problems across both the community and inpatient settings.Develop and strengthen celebrative partnerships and referral pathways with Indigenous Community Control Health Services and Non-government organisations.1 x Permanent Part Time Position (30.4 hours p.f.) at Bundaberg Hospital and1 x Permanent Part Time Position (30.4 hours p.f.) at Hervey Bay and Maryborough HospitalsPlease reference your preferred location on your application.Find out more in the Role Description.Our offer:You will receive a competitive remuneration package value up to $48,526 p.a., comprising salary rates $***** - $***** p.h.,
