Job Description
Kurri Kurri Community Services (KKCS) invites applications for an Enrolled Nurse Care Partner to join the Support at Home team in Loxford, NSW. You will coordinate care for older people in the community under clinical oversight from a Registered Nurse and work within a cooperative pod model.

You will conduct assessments, plan care, undertake home visits and maintain documentation while aligning with funding and individual goals, in a local not-for-profit setting.
