We are looking for an experienced Support Coordinator with mental health experience to join our team, on a part time basis with opportunity to move to full time.
n
This role is best suited to someone who:
n
Is confident working independently
n
Can manage their own caseload and priorities
n
Communicates clearly and follows through on tasks
n
Is comfortable working with complexity and uncertainty
n
Key Responsibilities
n
n
Support participants to understand and implement their NDIS plans
n
Coordinate services and liaise with multiple providers
n
Attend virtual and face to face meetings
n
Maintain accurate and compliant case notes and documentation
n
Support participants across a range of presentations, including, but not limited to, psychosocial, neurodivergent, and physical disabilities
n
Navigate crisis situations and system barriers where required
n
n
What We're Looking For
n
n
Experience in Support Coordination and/or Recovery Coaching (1+ years preferred)
n
Strong understanding of the NDIS
n
Ability to manage a caseload independently
n
Strong communication and documentation skills
n
Valid driver's licence and access to a vehicle
n
n

Lived experience (optional and welcomed)
