Our aim is for everyone to thrive in their role.About the roleProvide client-centred, evidence-based pharmacotherapy support as part of a multidisciplinary team.
Collaborate with GPs to assess substance use, advise on treatment options, facilitate access to care, coordinate health services, deliver health education, and provide basic first aid to support holistic client outcomes.Deliver high quality alcohol and other drug (AOD) administrative services within the health centreCollaborate with GPs and use dedicated clinic software programs to identify clients and facilitate care planning for clients who would benefit from EPC interventionsSupport care coordination aimed at the achievement of client goalsUndertake regular liaison with pharmacies,
