Care Partner - Support at Home

An opportunity is available for a Care Partner supporting residents within a retirement living and community care workplace. The position focuses on coordinating integrated care services, ensuring residents receive the appropriate support to maintain independence, wellbeing and quality of life.
n
This role works closely with residents, families, care teams and external providers to coordinate clinical and community-based services, provide medical assistance and ensure compliance with aged care standards and funding programs.

Key Responsibilities
n
n
Provide holistic case management for residents, promoting independence and enablement
n
Conduct pre-entry and ongoing assessments to determine care needs and appropriate support services
n
Develop, implement and regularly review comprehensive care plans
n
Liaise with residents, families and external providers to ensure continuity of care
n
Facilitate case conferences and risk assessments where required
n
Monitor and guide Care Service Workers to ensure services are delivered to the required clinical standards
n




Maintain accurate documentation and ensure compliance with aged care quality standards and regulatory requirements
n
Support resident wellbeing by encouraging participation in meaningful activities and social supports
n
Contribute to quality systems that support compliance with Aged Care Quality and Safety Commission requirements
n

About You
n
n
Minimum 1 years experience in a similar clinical or community care role
n
Understanding of Support at Home and relevant funding models
n
Strong knowledge of community aged care programs
n
Experience coordinating care within retirement living, community or residential aged care settings
n
Ability to build strong relationships with residents, families and multidisciplinary stakeholders
n
Strong documentation and administrative capability
n
Intermediate Microsoft Office skills
n
n
Qualifications and Requirements

Cert 3 or 4 min in Individual support
n
National Criminal History Check
n
Driving License
n
n

All Support at Home (SaH) package clients (~40 case loads)
n
Monday to Friday - 76 hours a fortnight
