Key Responsibilities

- Conduct home visits and build trusted relationships with members and families

- Complete care assessments, care plans, and ongoing reviews

- Identify changes in health, mobility, or support needs

- Coordinate services including domestic support, personal care, allied health, and social support

- Monitor member funding and ensure services align with package budgets





- The Care Manager will manage a small local team including a Care Coordinator and Rostering & Recruitment Coordinator, supporting them to deliver high-quality services to members

- Maintain accurate documentation and ensure compliance with aged care quality standards

- Support service quality and continuity of care for members

- Assist with the early growth of Maree Care’s presence in Adelaide

Who We’re Looking For

You’re someone who combines clinical understanding with strong relationship skills.

You may currently be working as a:

- Care Manager

- Care Coordinator ready to step up

- Clinical Lead within an aged care provider

You bring:

- Experience working within Home Care Packages / Support at Home

- Solid understanding of aged care services and funding structures

- Confidence conducting home visits and care planning

- Ability to build strong relationships with members and families

- Excellent organisation and documentation skills

Experience in community aged care is essential.
